Employee was dismounting electric pallet jack #12 when heel slipped on hydraulic fluid spill near dock leveler. Employee lost balance and fell backward onto concrete floor striking left wrist and lower lumbar.
Leaking high-pressure hose on pallet jack #12 was reported yesterday but not tagged out. Inadequate floor signage or spill absorption kit deployed.
Immediately red-tagged jack #12 for mechanic rebuild. Absorbent applied and dock degreased. Mandatory pre-shift hydraulic checklist retrained for all 3 shifts.
